Open Access Follow Up Nurse

About the role

The Open Access Follow Up Nurse provides high-quality, patient-centred care to individuals who have completed cancer treatment within Imperial College Healthcare NHS Trust. Working on stratified follow-up pathways, the role focuses on health promotion, self-management and holistic wellbeing, combining face-to-face assessments with telephone triage and remote monitoring.

Key responsibilities

• Deliver evidence-based nursing care to patients on stratified follow-up pathways.
• Conduct telephone triage for remotely monitored patients, escalating clinical concerns and ensuring timely investigations.
• Assess, plan, implement and evaluate individualized care plans, documenting accurately.
• Act as patient advocate, respecting needs and preferences.
• Collaborate with multidisciplinary teams, cancer CNS teams, Macmillan services and primary care providers.
• Provide health education to patients, relatives and carers, and support the development of support workers, nursing assistants and students.
• Participate in audits, quality-improvement projects and service development initiatives.
• Maintain professional standards in line with the NMC Code of Conduct and Trust policies.

Required profile

• Registered Nurse (NMC registration) with current licence to practise.
• Experience in cancer care or survivorship preferred.
• Ability to work autonomously and as part of a multidisciplinary team.
• Strong communication and documentation skills.
• Commitment to continuous professional development and evidence-based practice.
