Rehabilitation Community Nurse (Band 6) – Fixed‑Term

About the role

We are seeking a Band 6 Registered Nurse to join the North West Rehabilitation Service on a fixed‑term, part‑time basis. The role involves assessing, treating and supporting older and disabled adults with complex needs, promoting independence, self‑management and safe discharge from hospital.

Key responsibilities

• Manage a complex clinical caseload independently, applying specialist nursing knowledge.
• Deliver person‑centred rehabilitation care aimed at maximising independence.
• Collaborate with a multidisciplinary team to support assessment, treatment planning and discharge.
• Supervise support workers and nursing students.
• Provide professional nursing leadership, guidance and development within the team.

Required profile

• Registered with the Royal College of Nursing (RCN).
• Experience in community nursing and multidisciplinary teamwork.
• Relevant background in neurology.
