Sister/Charge Nurse – Frailty Pathway

About the role

This internal posting seeks a motivated Junior Sister to join the Frailty Pathway, including the Same Day Emergency Assessment Area. You will work within a supportive multidisciplinary team, delivering patient‑centred care to frail and elderly patients in a fast‑paced environment.

Key responsibilities

• Provide clinical leadership and support to nursing staff and support workers.
• Coordinate patient flow to ensure timely assessment, treatment, and discharge.
• Deliver safe, effective, and compassionate care to frail and elderly patients.
• Supervise, mentor, and develop junior staff.
• Collaborate with the wider multidisciplinary team to achieve optimal patient outcomes.
• Maintain high standards of care, safety, and documentation.
• Act as a role model, promoting a positive and proactive team culture.

Required profile

• Current employee of Hywel Dda University Health Board.
• Qualified nurse with experience in acute or community settings.
• Ability to work in a dynamic, fast‑moving environment.
• Desirable: ability to speak Welsh.
