Health Care Support Worker – AHP Long Term Conditions Service

About the role

The Long‑Term Conditions service is an AHP‑led, digital‑first self‑management programme delivering care to patients with chronic conditions such as ME/CFS, fibromyalgia, post‑viral fatigue and Long Covid. The role is hybrid, combining remote work from home, community home visits and attendance at a local base in Rowan Business Park or other health centres.

Key responsibilities

• Provide direct patient care and rehabilitation, delivering delegated self‑management treatment and rehabilitation programmes virtually or in the patient’s home.
• Adjust treatment plans within agreed parameters and report any unexpected changes in progress.
• Support patients experiencing complex fatigue, pain, breathlessness and reduced quality of life.
• Assist in the delivery of a 6‑week virtual self‑management group and specialised workshops.
• Develop Wellness Recovery Action Plans (WRAP) for patients on discharge.
• Perform manual handling and safely use mobility/therapy equipment.
• Implement therapeutic activities such as memory strategies, relaxation, anxiety and fatigue management.

Required profile

• Qualified Health Care Support Worker meeting HCPC standards or equivalent professional registration (CSP or RCOT).
• Valid driving licence and access to transport for visits across the NHSGGC area.
• Compliance with NHSGGC mandatory induction standards, policies and professional codes of conduct.
• Ability to work both remotely and in community settings.
