Staff Nurse – Hospital at Home

About the role

This permanent full‑time position is for a registered community staff nurse to join the Hospital at Home service operating from the Royal Alexandra Hospital. You will deliver acute level care to frail patients in their own homes, working within a virtual ward that operates 7 days a week.

Key responsibilities

• Assess, plan and implement individual care programmes for patients admitted to the virtual ward.
• Utilise clinical and environmental risk‑assessment tools to identify and mitigate potential hazards.
• Provide acute interventions, medication administration and monitoring in patients' homes.
• Collaborate with the lead Advanced Nurse Practitioner (ANP) and multidisciplinary team to ensure safe, high‑quality care.
• Support the expansion of the service to an 18‑bed model, including shift work from 8 am to 8 pm, five days a week.

Required profile

• Current registration with the UK Nursing and Midwifery Council (NMC) and a live NMC pin.
• Valid driver’s licence and access to a car.
• Willingness to work shift patterns covering public holidays.
• Experience in community or acute nursing, preferably with frailty pathways.

Required skills

• Clinical assessment and decision‑making.
• Use of risk‑assessment tools.
• Delivery of acute care interventions in a home setting.
