Support Coordinator – Stroke Recovery Service

About the role

This part‑time, home‑based position supports stroke survivors and their carers across Birmingham and Solihull. Reporting to the Stroke Association Service Delivery Coach, you will travel extensively to deliver personalised advice, assessments and recovery planning.

Key responsibilities

• Collaborate with local health and social care services to receive and manage referrals.
• Provide tailored information, advice and ongoing support to stroke survivors and their families.
• Conduct needs assessments and help develop individual stroke recovery plans.
• Carry out regular reviews to monitor progress and assist clients in achieving personal goals.

Required profile

• Background in a caring or charitable role supporting people with disabilities.
• Proven experience working collaboratively with multiple organisations.
• Experience supporting individuals with additional communication needs.
• Ability to travel widely across the region.

Required skills

• Proficiency in Microsoft Word.
• Proficiency in Microsoft Excel.
