Charge Nurse – Supported Discharge Unit (Band 6)

About the role

The Supported Discharge Unit at NHS Fife is seeking a Band 6 Charge Nurse to provide leadership and direction across the unit and associated areas. This role supports safe patient discharge, collaborates with multidisciplinary teams, and drives continuous improvement.

Key responsibilities

• Lead and support the Senior Charge Nurse in day‑to‑day operations of the Supported Discharge Unit.
• Provide Charge Nurse cover for the Nursing Pool, Discharge Lounge and Ward 9.
• Coordinate with the Discharge HUB and Social Work to identify discharge pathways and set Planned Date of Discharge (PDD).
• Facilitate MDT meetings to maintain site capacity and patient flow.
• Champion change management initiatives to achieve high standards of care.

Required profile

• Post‑registration nursing experience, preferably within discharge or acute care settings.
• Demonstrated leadership and management abilities.
• Excellent interpersonal and communication skills.
• Ability to handle complex discharges and challenging conversations.
