About the role

The AHP Long‑Term Conditions Service delivers a digital‑first, six‑week virtual self‑management programme for patients with chronic health conditions such as ME/CFS, fibromyalgia, post‑viral fatigue and Long Covid. As an Occupational Therapist you will provide specialist interventions, both remotely and through community home visits, supporting patients to improve function and quality of life.

Key responsibilities

  • Deliver occupational therapy interventions within virtual groups, workshops and one‑to‑one sessions.
  • Manage a specialist clinical caseload across six health and social care partnerships.
  • Oversee daily operations of the Occupational Therapy service within the AHP team.
  • Educate and develop junior OT staff and multidisciplinary colleagues.
  • Participate in research and development activities related to long‑term condition management.
  • Provide specialist advice and resources to patients, carers and other health‑care agencies.

Required profile

  • Degree in Occupational Therapy (or equivalent) and HCPC registration.
  • Demonstrated community experience with a range of long‑term conditions.
  • Valid driving licence and access to a car for regional visits.
  • Ability to work autonomously within a board‑wide, hybrid service model.
