Open Access Follow Up Nurse

About the role
The Open Access Follow Up (OAFU) Nurse provides high‑quality, patient‑centred care to individuals who have completed cancer treatment at Imperial College Healthcare NHS Trust. Working within stratified follow‑up pathways, the role focuses on health promotion, self‑management and holistic wellbeing, collaborating with multidisciplinary teams, cancer CNS teams, Macmillan services and primary care.
Key responsibilities
- Deliver evidence‑based nursing care to patients on stratified follow‑up pathways.
- Conduct telephone triage and remote monitoring, escalating clinical concerns as needed.
- Assess, plan, implement and evaluate individualised patient care plans.
- Act as patient advocate, ensuring preferences and needs are respected.
- Maintain accurate patient records and ensure compliance with Trust policies.
- Communicate effectively with the multidisciplinary team to support coordinated care.
- Identify information and support needs of patients, relatives and carers, providing health education where required.
- Support the learning and development of cancer support workers, nursing assistants and students.
- Participate in audits, quality‑improvement initiatives and personal development planning.
Required profile
- Registered Nurse (NMC‑registered) with a genuine interest in cancer care and survivorship.
- Commitment to delivering compassionate, patient‑focused care within a multidisciplinary environment.
- Ability to maintain professional standards in line with the NMC Code of Conduct and Trust policies.
