Qualified Social Worker – Neighbourhood Team (Bolton)

About the role

We are seeking an experienced Qualified Social Worker to join the Neighbourhood Team supporting Bolton Council. You will work alongside a multi‑disciplinary team to deliver a coordinated approach to meeting the needs of individuals in local neighbourhoods.

Key responsibilities

• Conduct strengths‑based assessments under the Care Act 2014.
• Develop and implement comprehensive support plans, linking service users to universal and voluntary services.
• Carry out carers assessments and formulate support plans in line with the Care Act 2014.
• Apply the principles of the Mental Capacity Act 2005 and other relevant legislation.
• Participate in a busy duty rota, taking referrals and providing information and advice at the point of contact.
• Collaborate with multi‑disciplinary colleagues to ensure a joined‑up service delivery.

Required profile

• Qualified Social Worker with proven experience in the Care Act 2014 assessments.
• Strong knowledge of the Mental Capacity Act 2005 and other social work legislation.
• Ability to work flexibly within a duty system and on a hybrid working model.
• Excellent communication skills for interacting with service users and partner agencies.
