Secure Pharmacy Technician – Medicines Reconciliation & Safety

About the role

This position supports a safe, patient‑centred pharmacy service for young people in a secure environment. Working under the guidance of a pharmacist and chief pharmacist, you will ensure medicines are reconciled, optimised and used safely in line with national standards and local policies.

Key responsibilities

• Undertake comprehensive medicines reconciliation for new admissions and transfers, identifying and resolving discrepancies.
• Perform medication checks to optimise therapy, reduce polypharmacy and support patient adherence.
• Assist with Medicines Use Reviews (MURs) and provide individualised counselling on medication use and side effects.
• Recognise and escalate signs of drug or alcohol withdrawal, supporting patients at risk of self‑harm.
• Conduct regular audits, monitor formulary compliance and provide data for KPI and contract performance reports.
• Promote medicines safety, maintain controlled‑drug records and ensure secure custody protocols are followed.
• Collaborate with multidisciplinary teams, attend clinical meetings and contribute to governance activities.

Required profile

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
• Relevant NVQ Level 3 / BTEC in Pharmacy Services or equivalent qualification.
• Desirable: accreditation in medicines reconciliation or accuracy checking.
• Strong organisational and prioritisation abilities to manage workload in a challenging environment.
