Adult Social Worker – Transitions Team

About the role

The Transitions team at Herefordshire Council is seeking an Adult Social Worker to support young adults aged 18‑25 as they move from children’s services into adult social care. You will work in line with the Care Act 2014 to promote wellbeing and independence.

Key responsibilities

• Carry out strengths‑based assessments and identify eligible needs for young adults.
• Develop and review personalised care and support plans.
• Conduct Section 42 safeguarding enquiries and CHC assessments.
• Perform Mental Capacity Act assessments and best‑interest decision‑making.
• Contribute to court work and liaise with families and multi‑agency partners.

Required profile

• Qualified Adult Social Worker, registered with the appropriate professional body.
• Experience working with 18‑25‑year‑old clients and multi‑agency environments.
• Knowledge of the Care Act 2014, Section 42, CHC and the Mental Capacity Act.
• Willingness to undergo an Enhanced DBS disclosure.
