Care Coordinator (Administrator)

About the role

As a Care Coordinator (Administrator) you will join the Lancashire Health Visiting and School Nursing Service, supporting the delivery of community health services for children and families. You will work within HCRG Care Group’s multidisciplinary team to ensure smooth administrative processes and high‑quality care coordination.

Key responsibilities

• Maintain and update patient and service records, ensuring accuracy and confidentiality.
• Schedule appointments, manage referrals and liaise with health visiting and school nursing staff.
• Provide administrative support for the delivery of health visiting and school nursing programmes.
• Assist with reporting, data entry and preparation of service performance metrics.
• Respond to enquiries from families, schools and partner organisations, delivering timely information.

Required profile

• Good organisational and communication skills with a customer‑focused attitude.
• Ability to work collaboratively within a multidisciplinary health team.
• Attention to detail and ability to handle confidential information responsibly.
