About the role
We are seeking an experienced Children’s Social Worker to join the Leaving Care Team in Aylesbury. The role focuses on supporting Looked‑After Children as they transition out of care, ensuring their wellbeing and successful integration into independent living.
Key responsibilities
- Work within the Leaving Care Team to provide case management for Looked‑After Children.
- Develop and implement transition plans for children leaving care.
- Collaborate with external agencies and professionals to coordinate support services.
Required profile
- Qualified social worker with experience in children’s social work.
- Demonstrated experience working with Looked‑After Children or in a Leaving Care setting.
