Diabetes Specialist Nurse – Possilpark Health & Care Centre

About the role

The Diabetes Specialist Nurse will join the Possilpark Health & Care Centre as an autonomous practitioner delivering specialist diabetes care to patients and their families. This permanent, full‑time position requires a current NMC pin and the ability to work within NHS guidelines while supporting the multi‑disciplinary team.

Key responsibilities

• Assess, plan, implement and evaluate individual care programmes for patients with diabetes.
• Develop and deliver complex diabetes management packages, including insulin therapy and sick‑day rules.
• Provide education and support to patients, carers and other healthcare professionals.
• Coordinate seamless care from admission to discharge, ensuring effective communication with the MDT.
• Supervise and mentor nursing staff, adhering to professional codes of practice.
• Drive to patient homes or community sites as required and obtain a PVG scheme clearance.

Required profile

• Current NMC registration (UK pin).
• Registered Nurse with post‑registration education in diabetes care.
• Significant experience in diabetes management, preferably at an advanced specialist level.
• Ability to work autonomously and make clinical judgments.
• Willingness to drive for work purposes and obtain PVG clearance.

Required skills

• Diabetes assessment and monitoring.
• Insulin therapy initiation and adjustment.
• Glycaemic control planning and target setting.
• Patient education on lifestyle and self‑management.
• Coordination of multidisciplinary care pathways.
