Experienced Social Worker – Adult Community Team

About the role

Join Wiltshire Council's Adult Community Team as an Experienced Social Worker, supporting adults with physical disabilities, chronic conditions, autism or sensory impairments. You will work in a compassionate, person‑centred environment that values empathy, respect and integrity.

Key responsibilities

• Conduct Care Act 2014 assessments and develop personalised care plans.
• Identify and manage short‑term and long‑term cases, coordinating with families, carers and multidisciplinary partners.
• Progress safeguarding enquiries and support Court of Protection matters.
• Collaborate with health professionals, social services and other agencies to ensure holistic support.
• Adapt quickly to changing needs while maintaining professional standards.

Required profile

• Registered with Social Work England.
• Strong knowledge of the Care Act 2014 and safeguarding procedures.
• Demonstrated experience in adult social work, with a balanced approach to risk.
• Passionate about making a meaningful difference and contributing to best practice.
