MAP Social Worker

About the role

We are looking for a compassionate MAP Social Worker to join Connect2Surrey in Woking. The role provides vital support and advocacy for individuals facing unauthorised or precarious situations, helping them navigate health, housing and legal services with dignity.

Key responsibilities

• Provide holistic social work support to clients experiencing unauthorised challenges, ensuring safety and well‑being.
• Conduct comprehensive assessments to identify needs, strengths and barriers.
• Develop and implement personalised care plans in collaboration with multidisciplinary teams.
• Advocate for clients’ rights and facilitate access to healthcare, housing and legal assistance.
• Maintain accurate, confidential case records in line with organisational and legal standards.
• Build and sustain partnerships with community organisations, agencies and stakeholders.
• Deliver culturally sensitive interventions respecting diverse backgrounds.
• Participate in professional development, team meetings and service improvement activities.

Required profile

• Bachelor’s or Master’s degree in Social Work or a related field.
• Valid social work licence or eligibility for licensure.
• Experience working with individuals in unauthorised or precarious situations (preferred).
• Strong understanding of social justice, equity and inclusion principles.
• Excellent communication, interpersonal and advocacy abilities.
• Ability to work both independently and as part of a multidisciplinary team.
• Commitment to confidentiality and ethical practice.
• Cultural competency and sensitivity to diverse populations.
