Prostate Self‑Supported Management (SSM) Care Coordinator

About the role

This permanent Band 4 position supports the Prostate Clinical Nurse Specialist team at Royal Surrey County Hospital. The Care Coordinator will help patients navigate the Prostate Cancer Supported Self‑Management pathway, providing information, monitoring, and emotional support.

Key responsibilities

• Act as the main point of contact for patients on remote monitoring pathways.
• Coordinate PSA tracking, safety‑netting processes and follow‑up appointments.
• Provide high‑quality information, sign‑posting and practical support to patients, carers and families.
• Complete holistic assessments and develop personalised care plans.
• Refer complex cases to the Clinical Nurse Specialist team and assist with delegated clinical tasks.
• Monitor patients on active surveillance, including blood tests and MRI scans.
• Support the prostate support worker in delivering Holistic Needs Assessments.

Required profile

• Motivated, organised and compassionate.
• Excellent communication skills and ability to handle sensitive information.
• Capability to work autonomously and as part of a multidisciplinary team.
• Previous oncology experience is an advantage.
