Qualified Social Worker - Adult Triage

About the role

This position is based at the front door of adult social care within an integrated health and social care team in Bexley. The qualified social worker will lead rapid assessments, mental capacity evaluations and telephone triage, supporting individuals in crisis and safeguarding situations.

Key responsibilities

• Conduct adult social care assessments and mental capacity assessments.
• Provide telephone triage and respond to urgent enquiries.
• Work on a duty rota, covering out‑of‑hours and crisis periods.
• Identify and act on safeguarding concerns.
• Collaborate with social care assistants, occupational therapists, nurses and physiotherapists.

Required profile

• Qualified social worker with registration.
• Ability to act quickly in crisis situations and perform dynamic risk assessments.
• Strong interpersonal skills and a collaborative team mindset.
