Rehabilitation Community Nurse – North West Rehabilitation Service

About the role

The North West Rehabilitation Service is seeking a Band 6 Registered Nurse to join a dynamic multidisciplinary team. The role focuses on assessment, treatment and intervention for older and disabled adults with complex needs, promoting independence and safe discharge.

Key responsibilities

• Manage a clinical caseload independently, applying specialist nursing knowledge.
• Deliver person‑centred rehabilitation care to maximise independence.
• Collaborate with a multidisciplinary team for assessment, treatment and discharge planning.
• Supervise support workers and nursing students.
• Provide professional nursing leadership, guidance and development within the team.

Required profile

• RCN‑registered nurse.
• Experience in community nursing and multidisciplinary teamwork.
• Relevant background in neurology.
