Remote Prescribing Pharmacist – Fully Remote

About the role

We are seeking a qualified Prescribing Pharmacist to join a fully remote, digitally‑enabled Hospital‑at‑Home service. The role involves conducting telephone‑based medication reviews for patients receiving care at home, optimising therapy and supporting safe prescribing.

Key responsibilities

• Conduct structured medication reviews with patients over the phone.
• Assess current medicines for safety, effectiveness and adherence.
• Amend, initiate or discontinue medications in line with clinical guidelines.
• Manage routine medication alerts and flag red‑flag concerns to clinical teams.
• Document all consultations and prescribing decisions accurately in clinical systems.
• Provide clear advice, reassurance and education to patients.

Required profile

• Registered Pharmacist with Independent Prescribing qualification.
• Experience delivering medication reviews in primary care, community or remote settings.
• Strong clinical decision‑making and risk‑assessment abilities.
• Confident telephone consultation style and ability to work autonomously.
• Excellent documentation, organisational and communication skills.
