Remote Prescribing Pharmacist – Fully Remote

About the role
We are seeking a qualified Prescribing Pharmacist to join a fully remote, digitally‑enabled Hospital‑at‑Home service. The role involves conducting telephone‑based medication reviews for patients receiving care at home, optimising therapy and supporting safe prescribing.
Key responsibilities
- Conduct structured medication reviews with patients over the phone.
- Assess current medicines for safety, effectiveness and adherence.
- Amend, initiate or discontinue medications in line with clinical guidelines.
- Manage routine medication alerts and flag red‑flag concerns to clinical teams.
- Document all consultations and prescribing decisions accurately in clinical systems.
- Provide clear advice, reassurance and education to patients.
Required profile
- Registered Pharmacist with Independent Prescribing qualification.
- Experience delivering medication reviews in primary care, community or remote settings.
- Strong clinical decision‑making and risk‑assessment abilities.
- Confident telephone consultation style and ability to work autonomously.
- Excellent documentation, organisational and communication skills.
