Support Coordinator – Supported Living

About the role

We are seeking a Support Coordinator to lead our Supported Living and outreach service in Blackpool. The role focuses on delivering person‑centred support to adults with learning disabilities, including those with autism spectrum conditions and complex needs.

Key responsibilities

• Develop independent living skills and promote community inclusion for service users.
• Identify and arrange activities that support valued and meaningful lifestyles.
• Provide respectful personal care, emotional and practical support.
• Collaborate with service users, families and the multi‑disciplinary team to maximise outcomes.
• Implement Creative Support policies and ensure compliance with CQC requirements.
• Supervise and support staff to meet service user needs and preferences.
• Support service users with challenging behaviour and apply positive behaviour support strategies.

Required profile

• Experienced practitioner with a background in learning disability support.
• Management experience supervising staff.
• Access to a car and willingness to work flexibly, including evenings and weekends.
• Commitment to person‑centred care and adherence to regulatory standards.
