Support Worker - Discharge to Assess

About the role

The British Red Cross is recruiting a Support Worker for its regulated service in Dundee, working with referrals from health professionals to provide practical and emotional support to vulnerable adults. The aim is to help prevent hospital re-admissions and promote wellbeing by helping people regain confidence and independence. The role is based in Dundee with travel around the area.

Key responsibilities

• Visit service users in their homes to assess needs and create tailored support plans
• Assist with daily living activities such as bathing, dressing and meal preparation
• Liaise with health, housing and social care professionals to ensure consistent support
• Carry out welfare checks and help individuals rebuild their independence

Required profile

• Experience supporting people with physical, sensory or mental health needs
• Knowledge of NHS and social care services and how to access them
• Flexible approach to working hours, including evenings and weekends
• Registration with the Scottish Social Security Council (SSSC) within 3 months of starting; sponsorship is not offered

• Salary of £24,479 per annum, pro rata for roles under 35 hours (£13.45 per hour); 35 hours per week, with 18 and 24-hour options
