Prostate Self‑Supported Management Care Coordinator

About the role

The Prostate Self‑Supported Management (SSM) Care Coordinator will join the Prostate Clinical Nurse Specialist team at Royal Surrey County Hospital. This Band 4 position supports the Prostate Cancer SSM pathway, providing patient‑centred care and coordination.

Key responsibilities

• Act as the primary contact for patients on remote monitoring pathways.
• Coordinate PSA tracking, safety‑netting processes and follow‑up appointments.
• Deliver high‑quality information, support and sign‑posting to patients, carers and families.
• Complete holistic assessments and develop personalised care plans.
• Assist the Clinical Nurse Specialist with delegated clinical and administrative tasks.
• Monitor patients on active surveillance, including blood tests and MRI scans.
• Support the prostate support worker in delivering Holistic Needs Assessments.

Required profile

• Highly motivated, organised and compassionate.
• Excellent communication skills and ability to manage a large patient group.
• Previous oncology experience is advantageous.
• Ability to work independently and as part of a multidisciplinary team.
