FEAT Sister – Frail Elderly Discharge Coordinator

About the role
The FEAT Sister will lead the coordination and discharge of frail elderly patients across multiple acute areas, working closely with the FEAT team to deliver comprehensive geriatric assessments and ensure safe, patient‑centred transitions of care.
Key responsibilities
- Coordinate discharge planning for frail elderly patients on MAU, ED, and wards 101, 306, 204, and fSDEC.
- Conduct comprehensive geriatric assessments in partnership with multidisciplinary health and social care teams.
- Identify and flag frail older patients on admission, ensuring accurate documentation.
- Develop, implement, and evaluate individualized discharge plans using current research evidence.
- Maintain effective communication with nursing, medical, and allied health professionals.
- Provide compassionate support to patients, relatives, and carers, respecting dignity and privacy.
- Record all discharge planning activities promptly and accurately.
- Utilise the electronic WhiteBoard to monitor safe discharge and patient transfers.
- Prioritise workload and promote effective time‑management within the FEAT team.
Required profile
- Registered Nurse with experience in acute or geriatric care.
- Demonstrated ability to work collaboratively within multidisciplinary teams.
- Strong organisational and prioritisation skills.
- Commitment to delivering high‑quality, patient‑focused care.
Required skills
- Proficiency with electronic patient record systems and WhiteBoard tools.
