Young Person Support Worker

About the role

We are seeking a dedicated Young Person Support Worker to join our team in Lewisham. The role focuses on helping care leavers aged 16‑25 transition to independent living, providing practical assistance and emotional support.

Key responsibilities

• Manage a caseload of young people, delivering regular key‑work sessions.
• Support development of independent living skills such as budgeting, cooking, tenancy management and accessing education or employment.
• Complete support plans, risk assessments and maintain up‑to‑date case notes.
• Promote wellbeing, safeguarding and personal development.
• Assist clients in accessing benefits, healthcare, training and community services.
• Collaborate with local authorities, social workers and external agencies.

Required profile

• Previous experience working with young people, care leavers or vulnerable adults.
• Knowledge of safeguarding and risk management procedures.
• Experience in preparing support plans and maintaining case records.
• Strong communication and relationship‑building abilities.
• Ability to work independently and manage a varied caseload.
