Community Stoma Care Nurse - Merseyside

About the role

Coloplast is looking for an experienced Registered General Nurse to deliver community Stoma Care services in the Merseyside area, working alongside partners at Royal Liverpool University Hospital and supporting patients in the surrounding areas. The nurse will act as an independent practitioner, self-motivated and able to think strategically.

Key responsibilities

• Deliver professional and ethical care to patients
• Support the patient pathway from hospital to home, providing home visits and community clinics
• Plan, manage and organise caseload and time effectively
• Communicate with patients, carers and referring healthcare professionals and give feedback
• Carry out an Appliance Use Review (AUR) where appropriate, following company and DoH guidelines
• Maintain clinical records in line with company, legal and professional requirements
• Build collaborative working between acute and community teams and with healthcare professionals across the region
• Provide educational support to colleagues and carry out service audit and evaluation

Required profile

• Registered General Nurse
• Nursing degree qualification desirable
• Expert clinician in stoma management
• Teaching experience

Required skills

• Sound interpersonal and relationship-building skills
• IT proficiency
