Urgent Care Practitioner (Out of Hours Telephony Triage)

About the role

Primary Care 24 is looking for Urgent Care Practitioners to join its out-of-hours team in Liverpool, providing telephone assessments of patients with clinical need. Positions are available in full-time, part-time or bank roles, with shifts Monday to Friday (6-hour shifts, 18:00 to midnight) and a 24/7 service over the weekend. The contract is based at Wavertree Technology Park.

Key responsibilities

• Liaise with the nominated medical and nursing leadership team to oversee patients awaiting contact.
• Manage clinical demand with the Clinical Coordinator or Shift Manager to meet agreed waiting times.
• Escalate concerns and priority patients to the Clinical Coordinator or on-call teams.
• Review patients awaiting contact, filtering for signposting to more appropriate services and applying a red flag approach to prioritise the elderly, palliative patients and those at risk of deterioration.
• Ensure safe transfers between clinical dispositions and signpost when clinically safe.
• Assess patients by telephone consultation, using professional clinical judgement supported by clinically based algorithms.

Required profile

• Extensive experience in emergency health care settings.
• Degree in Clinical Practice with a minimum of 3 years post-qualification experience.
• Experience of working in urgent care, primary care or a similar setting.

Required skills

• Significant expertise in clinical triage.
• Clinical Examination and Diagnostics Level 6/7, or relevant telephony triage experience.

What we offer

• Salaried: £49,583 per annum (pro rata); bank: £25.36 per hour, with bidding on shifts.
