Healthcare Support Worker – Upper Annandale Community Nursing Team

About the role

We are seeking an enthusiastic Healthcare Support Worker to join the Upper Annandale Community Nursing Team. You will help deliver high‑quality nursing care to housebound patients, supporting them 24 hours a day in a dynamic, multidisciplinary environment.

Key responsibilities

• Provide direct care and assistance to housebound patients in their homes.
• Collaborate with the Home Team, including allied health professionals, social workers and public health staff.
• Support the community nursing team in delivering safe, compassionate care.
• Adapt to changing priorities and respond to urgent patient needs.
• Maintain accurate records of patient interactions and care provided.

Required profile

• Enthusiastic and passionate about community health care.
• Ability to work in an ever‑changing environment.
• Experience in a community setting is preferred but not essential.
• Current driving licence (or alternative transport arrangements if a disability prevents it).
• Availability for a part‑time role of 25 hours per week.

Required skills

• Valid driving licence (or suitable alternative transport).
