Outreach and Sustainment Officer – Housing (East London)

About the role

We are seeking an experienced Outreach and Sustainment Officer to join our dedicated housing team in East London. You will work directly with people who are sleeping rough or at risk of homelessness, providing trauma‑informed outreach and support to help them secure and maintain stable accommodation.

Key responsibilities

• Deliver proactive outreach to identify and engage rough sleepers or those at risk.
• Conduct assessments of housing, support and welfare needs.
• Provide advice, guidance and sign‑posting to housing, health, welfare and specialist services.
• Support individuals moving into temporary accommodation, supported housing or independent living, ensuring successful settlement.
• Develop person‑centred support plans focused on long‑term outcomes.
• Collaborate with internal teams and external partner agencies to coordinate effective support.
• Perform welfare and safety checks, managing risks appropriately.
• Maintain accurate case records in line with policies and procedures.

Required profile

• Experience working with people experiencing homelessness, rough sleeping or complex needs.
• Knowledge of housing legislation, homelessness services and safeguarding practices.
• Proven ability to carry out support needs and risk assessments.
• Trauma‑informed, person‑centred approach.
• Excellent communication and relationship‑building skills.
• Ability to work independently while managing a varied caseload.
• Strong organisational skills and accurate record‑keeping.
