Support Worker - Hospital Discharge

About the role

The British Red Cross is recruiting a Support Worker for its 7-day hospital discharge service in Sandwell, covering West Bromwich, Wednesbury, Tipton and Rowley Regis. You will work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence. The team includes two Support Workers, two Coordinators and a Service Manager.

Key responsibilities

• Respond to referrals and complete welfare calls and home visits.
• Attend hospital wards and discharge units to support patients.
• Provide practical and emotional support to aid recovery and independence.
• Assess needs, identify safeguarding concerns and accurately record case notes.
• Signpost individuals to appropriate services and community resources.

Required profile

• Full UK manual driving licence held for a minimum of 2 years, and willingness to drive service vehicles.
• Flexibility and confidence working in different locations across Sandwell; the service is open 7 days a week from 9am to 8pm.
• Rotas are provided 3 months in advance, and the pattern includes one weekend every three weeks.
