Band 6 Clinical Streamer – Urgent Treatment Centre

About the role

The Band 6 Clinical Streamer is a senior nursing professional responsible for rapid clinical assessment, risk stratification and streaming of patients attending the Urgent Treatment Centre (UTC). Working autonomously, the role ensures patients are directed to the most appropriate care pathway, supporting safe and efficient patient flow.

Key responsibilities

• Perform initial clinical assessment and first‑line diagnostics for adult and paediatric patients.
• Stratify risk and stream patients to GP‑led urgent care, minor injury/illness services, emergency care, community alternatives or self‑care.
• Identify acutely unwell or deteriorating patients early and initiate immediate interventions.
• Collaborate with GPs, emergency nurse practitioners, UTC nursing teams, ambulance services and community providers.
• Contribute to service development, clinical governance and continuous quality improvement.
• Work day and night shift patterns to provide 24/7 coverage.

Required profile

• Registered Nurse with a current NMC registration.
• Accountable for clinical streaming of adults and children in an urgent care setting.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to collaborative working with health and social care professionals.
• Understanding of health promotion, illness prevention and the roles of other health and social care providers.
