Learning Disability Charge Nurse – Coming Home Project

About the role

We are seeking two Learning Disability Charge Nurses to join the Coming Home Project team within NHS Scotland. The role supports individuals transitioning back to community living after out‑of‑area placements, ensuring safe and sustainable care.

Key responsibilities

• Provide specialist assessment, intervention and risk management for people with complex learning disability needs.
• Support four individuals returning from out‑of‑area placements, coordinating with providers to maintain safe community care.
• Collaborate with CAMHS, Children & Young People’s Services, education, social work and adult LD teams to sustain stable placements and prevent crises.
• Work as part of a multidisciplinary team to deliver person‑centred, high‑quality care.

Required profile

• Current NHS Scotland employee (internal vacancy).
• Registered Learning Disability Nurse with experience in community settings.
• Commitment to person‑centred care, quality and teamwork.
• Eligibility to work in the United Kingdom and willingness to undergo required pre‑employment checks (PVG scheme if applicable).
