Clinical Nurse Specialist – Community Palliative Care

About the role

The Trust is seeking a Clinical Nurse Specialist to join a B7 Community Palliative Care Nurse Team (CPCT) delivering patient‑centred, evidence‑based palliative and end‑of‑life care across south Derbyshire. The role combines direct clinical work with leadership, education, audit and research responsibilities.

Key responsibilities

• Take overall responsibility for a caseload of patients with palliative diagnoses, providing specialist nursing care.
• Deliver CNS expertise in symptom control, as well as social, emotional and spiritual support.
• Participate in, implement and evaluate symptom‑management strategies within a multidisciplinary team.
• Contribute to maintaining and developing a high‑quality service.
• Provide advice and training across functional boundaries and the wider health‑community network.
• Ensure delivery of professional standards of care in the specialist area.
• Engage in research and audit activities and support dissemination of findings.
• Maintain personal professional development in line with Trust and NMC requirements.

Required profile

• Prior experience working in specialist palliative medicine at UHDB.
• Registered with the Nursing and Midwifery Council (NMC).
• Flexible approach to working, including availability on weekends and bank holidays.
• Leadership, managerial and educational capabilities.
